  1. The engine failure exceeded the design and certification standards for engine rotor blade containment. However, the aircraft design precautions were successful in that the damage sustained by the aircraft was not excessive to the point where it prevented a successful emergency landing.
  2. In the investigative phase of accidents or incidents, it is important to recognize and be on guard for "red herrings" or preliminary findings/observations that may not be relevant and may detract from determining the true root cause and necessary corrective actions of the event.
